Position Overview
The Comprehensive Clinical Assessor will provide comprehensive clinical assessments for consumers entering OR transitioning within the service continuum of Youth Villages and work directly with the clinical in-home counselor in the development of the person-centered plan for services, natural supports, and crisis prevention activities.
Essential Duties and Responsibilities
Conduct face-to-face evaluation virtually or in-person in the office, community, or home and assess the individual’s presenting mental health, developmental disability, and/or substance-related conditions and symptoms, resulting in the issuance of a written assessment summarizing the clinical service recommendation for the development of the PCP and initiation of needed services.
Responsible for the coordination of care activities, which may include but are not limited to the following: written progress or summary reports, telephone communication, treatment planning processes, other activities jointly determined by the referring provider.
Gather biological, psychological, familial, social, developmental, and environmental dimensions and identify strengths, weaknesses, risk factors, and protective factors in each area for development of the person-centered plan.
Formulate a description of the presenting problems, including source of distress, precipitating events, associated problems or symptoms, recent progressions, and current medications.
Participate in weekly individual and team meetings.
Qualifications
Master's degree in Social Work, Drug and Alcohol Education or Counseling, Counseling, Psychology, Criminal Justice, Guidance Counseling, Human Services, Marriage and Family Therapy and
Comprehensive Clinical Assessors must have one of the following licenses in the state of North Carolina: LCSW, LCMHC or LMFT.
