Evaluate patients to determine whether their symptoms are the result of a physical illness, a combination of physical and mental, or a strictly psychiatric one. Determine and lead treatment plan.

 

 

Essential Duties and Responsibilities include the following. Other duties may be assigned:

  • Review, process, and respond to requests for medical records and disability documentation in compliance with HIPAA and applicable federal/state laws.
  • Evaluate requests to ensure appropriate documentation is included and complete.
  • Coordinate with physicians, clinicians, and other departments to gather required records or clarify documentation.
  • Prepare and issue formal responses, including completion of disability forms (e.g. Social Security, insurance requests).
  • Verify authorization forms and ensure all record requests have valid client or legal guardian consent.
  • Maintain accurate records of all requests, responses, and communications using electronic health record (EHR) systems and tracking tools.
  • Communicate professionally with clients, legal representatives, and external agencies to clarify requests or provide updates.
  • Monitor request turnaround times to meet regulatory deadlines and organizational standards.
  • Assist in audits, quality assurance, and compliance reporting related to medical records access and release of information (ROI).
  • Ensure strict confidentiality and security of client information at all times.

 

Qualifications

Requirements

 

High School diploma or equivalent
1-3+ years of experience in medical records or health information management preferred
